Physicians and Practitioner Biographies
David Kosh Dr. Kosh was born and raised in Chicago, Illinois, where he attended Northwestern University, majoring in biology and anthropology. He continued at Northwestern University Medical School, attending on a Navy scholarship, and graduated in 1975. While at Northwestern he was selected for membership to Alpha Omega Alpha, a medical honorary society which admits only the top 10% of each medical school class. Initially board certified in Family Practice in 1976, he continues to score in the 99th percentile on the re-certification exam taken every six years.

Following medical school he was a resident in Family Practice at the Naval Regional Medical Center in Jacksonville, Florida. In his last year of residency, he was Chief Resident, supervising 27 residents in Family Practice. Obstetrics was an integral part of his residency and by the time he left Jacksonville he had delivered over 600 babies.

Following residency, Dr. Kosh was assigned to the Naval Hospital in Long Beach, California. Upon arrival, he was sent on temporary duty to Adak in the Aleutian Island chain. During his stay in the Aleutians, Dr. Kosh was the only physician delivering babies in a radius of two thousand miles. After this temporary assignment, he returned to Long Beach and became Director of the Family Practice Department with a staff of twelve physicians.

In 1983, Dr. Kosh resigned his regular commission and took his family overseas as an embassy physician for the Department of State, serving for three years in Kinshasa, Zaire and four years in New Delhi, India. His duties overseas included providing family medical care for American government personnel and dependents and serving as a 'circuit rider' for embassies throughout the region.

Wanting to continue 'hands-on' patient care, Dr. Kosh declined an administrative post in Washington, D.C. and returned to California as a private physician. He assumed the practice of retiring doctor Peter Pavitt in 1990.

Married thirty years with four grown children, he lives ten minutes from Methodist Hospital in nearby Elk Grove.

Harry Yeates Dr. Yeates was born and raised in Kingston, Canada, but his ties to Northern California extend to years of annual family vacations at Lake Tahoe. He majored in chemical engineering at Queen's University, one of Canada's premier institutions. After graduation he traveled to Finland for an engineering position but found that his true passion was in caring for others, returning to Queen's to study medicine, graduating in 1994.

After medical school he completed residency training in family practice with an additional fellowship in family based obstetrics. His unique talents made him sought by teaching programs, bringing him to Memphis where he has spent the last four years as a professor in a family practice residency. As Medical Director of the University of Tennessee Family Practice residency in Covington Tennessee, he taught residents care for patients in all age groups, in the office, at the hospital and at nursing homes. He was so well respected that he was nominated to be the Chief of the Medical Staff at Baptist Memorial Hospital in Tipton, Tennessee.

A desire for on hand individual care of patients led him to move to Sacramento. Married, with three small children, he enjoys running and cycling. In the winter, when not seeing patients, you will find him snowboarding at Tahoe.

Andy Jacobson Dr. Jacobson was born and raised on a dairy farm in west central Minnesota. He graduated from Hamline University with honors in physical chemistry. He attended the University of Minnesota medical school, where he developed a keen understanding of the life and work of family doctors.

Dr. Jacobson completed a three year residency in family practice at St. Francis Hospital in Wilmington, Delaware. During his last year he was chief resident, supervising 17 others in the program. Dr. Jacobson also had a year of general surgery training before his residency in family practice.

He then moved to Rochester, Minnesota and Pittsburgh, Pennsylvania so his wife, also a physician, could complete felllowships in her area of specialty. Dr. Jacobson was able to hone his skills in acute care as he worked at various hospitals and offices caring for a wide range of patients.

Dr. Jacobson is looking forward to his new life in Sacramento and establishing roots in this area. He enjoys running and golfing year round, and winter will find him on the ski slopes or relaxing by the Pacific coast. He and Laurie, his wife of 11 years, have one horse, two Italian greyhounds, and two cats.

Micheal Smith Micheal was born and raised in California, attending community college before joining the U.S. Navy. As a hospital corpsman stationed in San Diego and Oakland, he worked as a nursing corpsman on surgical and ICU wards. Micheal also participated on a surgical support team during the 1975 evacuation of Vietnam. Upon returning to civilian life, he trained at Stanford as a certified paramedic, and firefighter with Washington and California fire departments. After completing a degree from Saint Mary’s college, he was accepted as a Paramedic field coordinator for a Northern California EMS agency. Returning to patient care as a licensed vocational nurse in hospital and office practice, Micheal subsequently completed U.C. Davis’s FNP/PA program.

Micheal has since gained urban and rural PA experience in family practice, urgent care, occupational health, and correctional medicine. He is a board-certified PA, on the volunteer faculty at U.C. Davis School of Medicine and a member of the California and American Association of Physician Assistants. Micheal’s interest encompasses general primary care including geriatrics and pulmonary medicine. He looks forward to the privilege of helping you with your medical care.

Micheal lives in Davis with his family, enjoying raising two spirited boys, camping and riding bicycles.

LUZ ALCARAZ Luz grew up in the Sacramento Delta area. She attended the University of San Francisco, a Catholic and Jesuit institution, as an undergraduate majoring in nursing. As an undergraduate, she volunteered in the campus ministry. After obtaining a Bachelor of Science Degree in Nursing, she returned to Sacramento.

From 1987 to 1997, she was employed at Sutter General Hospital, where she worked in telemetry, emergency medicine, and floating to the intensive care unit and other hospital units. In addition to working full time, she volunteered educating the Hispanic population on multiple health topics.

Luz has been a family nurse practitioner since 1997. She completed the family nurse practitioner/physician assistant program at UC Davis School of Medicine following the Masters track. She also completed a Masters of Science and Nursing, a combined program at the UCD FNA/PA program and Sacramento State University. Luz completed her Masters thesis on diabetes management in the adult Hispanic population. She conducted her research at a local outpatient primary care clinic. She enjoys all aspects of family practice since it allows for a direct involvement in health education and promotion.

In her spare time, Luz enjoys jogging, swimming and bicycle riding with her husband and their nine-year-old daughter. She enjoys her roles as wife and mother.

Jolene Woodley Jolene was born and raised in Billings, Montana. She graduated from Gonzaga University in Spokane, Washington with a degree in biology. While attending Gonzaga she worked part time as a nursing assistant in both nursing homes and assisted living facilities.

After graduation, Jolene traveled to the East Coast where she volunteered in the AmeriCorps National Civilian Conservation Corps for ten months. After learning to build bridges, trails and communities in the AmeriCorps, Jolene enlisted in the Air Force. As a medic in the Air Force she worked in the surgery/urology and family practice clinics. In addition to working as a medic, she coached a local junior Olympic volleyball team for two years.

In 2000, Jolene was accepted to attend the Physician Assistant program at Fort Sam Houston in San Antonio, Texas. She did her clinical training at Nellis AFB in Las Vegas. After her graduation and commissioning as an officer, Jolene was stationed at Travis AFB where she was assigned to work at the Mather and McClellan satellite clinics. While working at the McClellan clinic, Jolene was able to complete her master’s degree in PA Studies from the University of Nebraska. Jolene recently resigned her commission in order to remain in the Sacramento area where she could continue to practice family medicine and remain close to family and friends. Jolene enjoys running, mountain biking, skiing, camping and hiking, which help to make Sacramento home for a Montana girl.

REBECCA BRIGGS Rebecca joined Associated Family Physicians in spring 2006. She earned a Bachelor of Science in Nursing from the University of Maine and has worked in both hospital settings and outpatient clinics.

Rebecca joined the Air Force in 1995 and was assigned to Travis Air Force Base in Fairfield. She served four years as an active duty Air Force nurse, working on an inpatient surgical unit at David Grant Medical Center.

After her discharge from the Air Force, Rebecca became a member of the Air Force Reserve. She continued to work at Travis as a civilian, delivering post anesthesia care to active duty dependents and retired military post-operative patients.

Rebecca earned her Master's of Science in Nursing and Family Nurse Practitioner certification at Holy Names College in Oakland in 2003. In addition to her studies, she worked in a surgery clinic in Solano County.

She is particularly interested in helping patients achieve weight loss. She is an associate of the American Society of Bariatric Physicians and ran the weight loss program at New Dimensions Medical Group in Marysville.

Rebecca grew up on the New Jersey beaches and loves California -- "anything in or on the water," she says. Reading is her passion, especially biographies and historical fiction. Rebecca also enjoys movies and skiing. She is the mother of three grown sons.

ROYAL GEE Royal grew up in Sacramento and has lived here most of his life. He completed his undergraduate degree in Health Science/Occupational Health at California State University Sacramento, and continued his training at Kaiser Permanente, focusing on employee injury and illness prevention. He was a teaching assistant in physiology and anatomy at Cosumnes River College prior to pursuing his clinical education. Soon after, he relocated to Seattle, Washington and enrolled in Seattle University, where he earned a Master of Science in Nursing degree, first becoming a R.N. and then a family nurse practitioner (FNP).

While in Seattle, he volunteered and worked with diverse populations, including non-English speaking, homeless, and gay and lesbian patients. He has published several articles on primary care health issues of underserved populations. His clinical experience has been in infectious disease, rural health, international community health, and mental health. Royal enjoys all aspects of family medical care, especially infectious diseases and health education.

When not at work, Royal enjoys live theatre and cinema and spends long days at the park with his basset hound. He currently resides in Sacramento.
