Galt office now open
Associated Family Physicians has expanded to meet our patients' needs. Our Galt office opened in October 2004 at 448 C Street (next to Spaan's Cookies). Office hours are 8:00am-5:00pm.
Weekend Hours
Our Timberlake office is open Saturday and Sunday from 9:00am-5:00pm by appointment. Family nurse practitioners and physician's assistants rotate weekends for routine and urgent care appointments. The doctors are available for hospital admissions and to assist the providers with difficult cases on weekends.

Health Care Providers
Dr. Harry Yeates joined our practice in October 2003. Dr. Yeates completed a residency in family practice as well as an additional fellowship in obstetrics. Born in Canada, he attended medical school at Queen’s University in Toronto, graduating in 1994. Dr. Yeates was director of the family practice residency program in Memphis, Tennessee before moving to Sacramento.
Dr. Andy Jacobson started his AFP practice in fall 2005. A Minnesota native, he attended the University of Minnesota medical school and completed a three year residency in family practice at St. Francis Hospital in Wilmington, Delaware. During his last year he was chief resident. Dr. Jacobson moved to Sacramento from Pittsburgh, where his wife was completing fellowships in her medical specialty.
